What gynecomastia is, and who surgery helps

Gynecomastia refers to the development of excess breast tissue in men, whether glandular, fatty, or a mix of both. It can affect one or both sides of the chest and is common across a wide age range. In many cases it causes discomfort or self-consciousness, particularly when exercising or wearing fitted clothing.

Surgery is considered when the tissue does not resolve on its own and when non-surgical measures have not helped. A medical evaluation first rules out any underlying cause, such as a hormonal imbalance or medication effect. If surgery is suitable, the approach is chosen based on the proportion of glandular tissue versus fat. Suitability, technique and risks are assessed at consultation.

The right approach depends on whether the enlargement is mainly glandular tissue, mainly fat, or a combination. This is assessed before surgery.

Liposuction of the chest

When excess volume is predominantly fatty, liposuction through small incisions can reduce chest bulk and improve contour. Suitable where glandular tissue is minimal.

Glandular tissue excision

Firm glandular tissue under the nipple does not respond to liposuction and is removed directly through a small incision at the areola margin. This addresses the hard disc of tissue that causes a puffy or pointed nipple appearance.

Combined approach

Most cases involve both glandular tissue and excess fat. Combining excision with liposuction addresses both components in a single procedure, producing a flatter, firmer chest contour.

Recovery and what to expect

You will wear a compression vest for the first few weeks, with swelling and bruising that ease gradually. Most people return to a desk job within one to two weeks. Gym and upper-body exercise are generally avoided for around six weeks, following your surgeon’s guidance.

In-clinic LPG sessions can be arranged during the recovery phase to help manage swelling and support tissue settling after surgery.

What causes gynecomastia?

Gynecomastia has a range of causes, including hormonal changes during puberty or later in life, certain medications (such as anabolic steroids, some blood-pressure drugs, and others), weight gain, and in some cases no identifiable cause is found. A medical assessment before any surgical planning is important to identify or rule out a treatable underlying cause.

Is surgery necessary, or will liposuction alone be enough?

It depends on what the tissue is composed of. If the enlargement is mostly fat, liposuction may be sufficient. If there is a firm disc of glandular tissue under the nipple — which is common — excision is needed because liposuction cannot remove firm glandular tissue. Many cases benefit from both. This is determined at assessment.

Can gynecomastia come back after surgery?

Recurrence is uncommon after glandular tissue is fully excised. However, significant weight gain, hormonal changes or use of certain substances after surgery could contribute to the chest changing again. Maintaining a stable weight and avoiding known triggers reduces this risk. Results vary between individuals.

How long is recovery, and when can I return to the gym?

Most people return to a desk job within one to two weeks. Upper-body exercise and gym training are generally avoided for around six weeks, with your surgeon’s clearance before resuming. A compression vest is worn in the early weeks to support healing. Your surgeon provides a personalised timeline.

Will there be visible scars?

Liposuction leaves only small marks at the entry points. Glandular excision uses an incision along the lower edge of the areola, where it tends to fade and blend with the natural border.
